Self Reporting Form

Access the self-reporting form here.

On the first day back from isolation/quarantine, please print, complete and return to your child's teacher the Shortened Return Parent Form.

All positive individuals must stay home from school 5 days from the onset of symptoms or from the positive testing date. Based on current CDC guidance it is ok to end home isolation after testing positive for COVID-19 when:

  • Free from fever without the use of fever-reducing medications for at least 24 hours.
  • It has been at least 5 days since the onset of the patient's illness or positive diagnosis.
  • Symptoms improved

Vaccinated close contacts may remain in school as long as they do not have symptoms and with a negative test on day 5, 6, or 7 from the date of exposure.
Unvaccinated household close contacts must stay home 5 days from the last point of contact with a positive individual. Per CCDPH positive individuals should be encouraged to isolate from other household members in the home. A negative test will be required to return to school.

Unvaccinated non-household close contacts will be allowed to "test-to-stay" and may remain in school with negative tests on days 1,3,5 and 7 (or similar).  A school nurse will reach out to setup the test-to-stay schedule.
